The duty to protect patient welfare underpins undergraduate medical ethics and patient safety teaching. The current syllabus for patient safety emphasises the significance of organisational contribution to healthcare failures. However, the ongoing over-reliance on whistleblowing disproportionately emphasises individual contributions, alongside promoting a culture of blame and defensiveness among practitioners. Diane Vaughan’s ‘Normalisation of Deviance’ provides a counterpoise to such individualism, describing how signals of potential danger are collectively misinterpreted and incorporated into the accepted margins of safe operation. NoD is an (...) insidious process that often goes unnoticed, thus minimising the efficacy of whistleblowing as a defence against inevitable disaster. In this paper, we illustrate what can be learnt by greater attention to the collective, organisational contributions to healthcare failings by applying NoD to The Morecambe Bay Investigation. By focusing on a cluster of five ‘serious untoward incidents’ occurring in 2008, we describe a cycle of NoD affecting trust handling of events that allowed poor standards of care to persist for several years, before concluding with a poignant example of the limitations of whistleblowing, whereby the raising of concerns by a senior consultant failed to generate a response at trust board level. We suggest that greater space in medical education is needed to develop a thorough understanding of the cultural and organisational processes that underpin healthcare failures, and that medical education would benefit from integrating the teaching of medical ethics and patient safety to resolve the tension between systems approaches to safety and the individualism of whistleblowing. This essay argues for a two-part thesis concerning the deflationist theories of truth and reference. First, I identify two points of contrast between the deflationist theories and their traditional inflationary opponents: (1) they each employ different orders of explanation for the variety of semantic phenomena, and (2) the inflationist is typically taken to be beholden to a reductive explanation of reference, whereas the deflationist is doubtful of this project. Secondly, I argue that these two points of contrast need not come (...) together to exhaust the space of possible views. There is room for a plausible middle ground: a moderate version of inflationism. My moderate inflationism will reject the deflationist's order of explanation, so it counts as genuinely inflationary. However, it also rejects the reductionist ambitions of the earlier inflationists, so it isn’t an apt target for deflationary scepticism. (shrink)

Martin Drenthen has made a strong case for his interpretation of Nietzsche’s potential contribution to environmental ethics but he does not do justice to deep ecology. The problematic he identifies is essentially the difficulty of asserting a meaningful basis for action while being aware of the contingency of all meanings.This tension can be seen running through deep ecology, at least as described by its main theorist, Arne Naess, who is not the moral realist that Drenthen would have him. Key differences (...) do emerge as Drenthen develops his idea of “Wildness” or nature as “other,” which can only partially support caring action towards nature. Drenthen is ambivalent, even hostile, to a context of experienced reality – central to Naess’s ecosophy.This causes him to fall into what is ultimately a fairly traditional nature versus culture distinction and so maintain the existential gulf between humans and nature. Even though disquotationalism is not correct as it is usually formulated, a deep insight lies behind it. Specifically, it can be argued that, modulo implicit commitment to reflection principles, all there is to the notion of truth is given by a simple, natural collection of truth-biconditionals.

This paper attempts to explain what a protest is by using the resources of speech-act theory. First, we distinguish the object, redress, and means of a protest. This provided a way to think of atomic acts of protest as having dual communicative aspects, viz., a negative evaluation of the object and a connected prescription of redress. Second, we use Austin’s notion of a felicity condition to further characterize the dual communicative aspects of protest. This allows us to distinguish protest from (...) some other speech acts which also involve a negative evaluation of some object and a connected prescription of redress. Finally, we turn to Kukla and Lance’s idea of a normative functionalist analysis of speech acts to advance the view that protests are a complex speech act constituted by dual input normative statuses and dual output normative statuses. Recent studies of spider phobia have indicated thatfearof spiders is closely associated with the disease-avoidance response of disgust. It is argued that the disgust-relevant status of the spider resulted from its association with disease and illness in European cultures from the tenth century onward. The development of the association between spiders and illness appears to be linked to the many devastating and inexplicable epidemics that struck Europe from the Middle Ages onwards, when the spider was a suitable displaced target for (...) the anxieties caused by these epidemics. Such factors suggest that the pervasive fear of spiders that is commonly found in many Western societies may have cultural rather than biological origins, and may be restricted to Europeans and their descendants. Sudoku puzzles, which are popular worldwide, require individuals to infer the missing digits in a 9 9 array according to the general rule that every digit from 1 to 9 must occur once in each row, in each column, and in each of the 3-by-3 boxes in the array. We present a theory of how individuals solve these puzzles. It postulates that they rely solely on pure deductions, and that they spontaneously acquire various deductive tactics, which differ in their difficulty (...) depending on their “relational complexity”, i.e., the number of constraints on which they depend. A major strategic shift is necessary to acquire tactics for more difficult puzzles: solvers have to keep track of possible digits in a cell. We report three experiments corroborating this theory. We also discuss their implications for theories of reasoning that downplay the role of deduction in everyday reasoning. (shrink)

A theory of truth is language-transcendent if it ascribes truth conditions to truth-bearers that are not expressible in our natural language; a theory is language-immanent if it is not language-transcendent. In this paper, I argue for the following theses. Whether the correct theory of truth is language-transcendent or language-immanent will have significant consequences for general philosophy. Prima facie, a language-transcendent theory is preferable. However, language-transcendent theories tend to require substantive metaphysical commitments concerning truth. Deflationist theories are particularly interesting in this (...) context. I argue that, if deflationism is to remain consistent with its motivations, then it must opt for a language-immanent conception of truth. (shrink)

Hylomorphism is the Aristotelian theory according to which substances are composites of matter and form. If my house is a substance, then its matter would be a collection of bricks and timbers and its form something like a structure that unites those bricks and timbers into a single substance. Contemporary hylomorphists are divided on how to understand forms best, but a prominent group of theorists argue that forms are emergent powers. According to such views, when material components are arranged appropriately, (...) a novel substance emerges with the power to impose unity on its components through time. I argue that these accounts of form fall prey to a bootstrapping problem, and so, suffer from issues of redundancy, given plausible assumptions about inherence. In their place, I suggest an ontologically minimalist conception of forms as collective manifestations of the powers of matter. (shrink)

In contrast to many aspects of the social responsibility of business, CSR scholarship has been largely silent on the issue of the payment of corporate tax. This is curious because such tax payments are often considered a fundamental and easily measured example of a company’s citizenship behavior. However, because the payment of corporate tax can often be legally avoided, this activity represents a boundary condition for CSR. If the law and CSR suggest that a company should pay its fair share (...) of tax, yet many successful companies actively avoid this social obligation, should they be considered socially irresponsible. This paper explores the issue of tax avoidance and the implications it has for any credible definition and measure of CSR. It also highlights an aspect of corporate practice that is deliberately hidden from public view. (shrink)

Following recent developments in the literature on axiomatic theories of truth, we investigate an alternative to the widespread habit of formalizing the syntax of the object-language into the object-language itself. We first argue for the proposed revision, elaborating philosophical evidences in favor of it. Secondly, we present a general framework for axiomatic theories of truth with theories of syntax. Different choices of the object theory O will be considered. Moreover, some strengthenings of these theories will be introduced: we will consider (...) extending the theories by the addition of coding axioms or by extending the schemas of O, if present, to the entire vocabulary of our theory of truth. Finally, we touch on the philosophical consequences that the theories described can have on the debate about the metaphysical status of the truth predicate and on the formalization of our informal metatheoretic reasoning. Most phobias are focussed on a small number of fear-inducing stimuli (e.g., snakes, spiders). A review of the evidence supporting biological and cognitive explanations of this uneven distribution of phobias suggests that the readiness with which such stimuli become associated with aversive outcomes arises from biases in the processing of information about threatening stimuli rather than from phylogenetically based associative predispositions or “biological preparedness.” This cognitive bias, consisting of a heightened expectation of aversive outcomes following fear-relevant stimuli, generates and maintains (...) robust learned associations between them. Some of the features of such stimuli which determine this expectancy bias are estimates of how dangerous they are, the semiotic similarity between them and their aversive outcomes, and the degree of prior fear they elicit. Ontogenetic and cultural factors influence these features of fear-relevant stimuli and are hence important in determining expectancy bias. The available evidence does not exclude the possibility that both expectancy biases and specific evolved predispositions coexist, but the former can explain a number of important findings that the latter cannot. It is widely agreed that matter bears a mereological relationship to substance; the bricks and timbers are parts of the house. But with form things are more controversial. Is the structure of the bricks and timbers best (...) conceived as a part of the house, or is it related to the house in some non-mereological fashion? Kathrin Koslicki argues that substances have formal parts, that forms are best conceived as bearing a mereological relation to substances. This paper shows that her argument fails, given the traditional and plausible distinction between substances and accidental unities. I close with a brief suggestion for a non-mereological construal of forms. (shrink)
