Since their public launch, a little over a year ago, large language models (LLMs) have inspired a flurry of analysis about what their implications might be for medical ethics, and for society more broadly. 1 Much of the recent debate has moved beyond categorical evaluations of the permissibility or impermissibility of LLM use in different general contexts (eg, at work or school), to more fine-grained discussions of the criteria that should govern their appropriate use in specific domains or towards certain (...) ends. 2 With each passing week, it seems more and more inevitable that LLMs will be a pervasive feature of many, if not most, of our lives. It would not be possible—and would not be desirable—to prohibit them across the board. Generative AI programs can produce high-quality written and visual content that may be used for good or ill. We argue that a credit–blame asymmetry arises for assigning responsibility for these outputs and discuss urgent ethical and policy implications focused on large-scale language models.

Recent advances in medical and information technologies, the availability of new types of medical data, the requirement of increasing numbers of study participants, as well as difficulties in recruitment and retention, all present serious problems for traditional models of specific and informed consent to medical research. However, these advances also enable novel ways to securely share and analyse data. This paper introduces one of these advances—blockchain technologies—and argues that they can be used to share medical data in a secure and (...) auditable fashion. In addition, some aspects of consent and data collection, as well as data access management and analysis, can be automated using blockchain-based smart contracts. This paper demonstrates how blockchain technologies can be used to further all three of the bioethical principles underlying consent requirements: the autonomy of patients, by giving them much greater control over their data; beneficence, by greatly facilitating medical research efficiency and by reducing biases and opportunities for errors; and justice, by enabling patients with rare or under-researched conditions to pseudonymously aggregate their data for analysis. Finally, we coin and describe the novel concept of prosent, by which we mean the blockchain-enabled ability of all stakeholders in the research process to pseudonymously and proactively consent to data release or exchange under specific conditions, such as trial completion. Drugs used to provide improvement of cognitive functioning have been shown to be effective in healthy individuals. It is sometimes assumed that the use of these drugs constitutes cheating in an academic context. We examine whether this assumption is ethically sound. Beyond providing the most up-to-date discussion of modafinil use in an academic context, this contribution includes an overview of the safety of modafinil use in greater depth than previous studies addressing the issue of cheating. Secondly, we emphasize two crucial, (...) but hitherto nearly overlooked, nuances to the issues: the potential for modafinil to decrease inequality and disadvantage in academic settings, and the fact that how modafinil is used dramatically impacts its effects on health, coercion, fairness, authenticity and effort. When making substituted judgments for incapacitated patients, surrogates often struggle to guess what the patient would want if they had capacity. Surrogates may also agonize over having the (sole) responsibility of making such a determination. To address such concerns, a Patient Preference Predictor (PPP) has been proposed that would use an algorithm to infer the treatment preferences of individual patients from population-level data about the known preferences of people with similar demographic characteristics. However, critics have suggested that even if such (...) a PPP were more accurate, on average, than human surrogates in identifying patient preferences, the proposed algorithm would nevertheless fail to respect the patient’s (former) autonomy since it draws on the ‘wrong’ kind of data: namely, data that are not specific to the individual patient and which therefore may not reflect their actual values, or their reasons for having the preferences they do. Taking such criticisms on board, we here propose a new approach: the Personalized Patient Preference Predictor (P4). The P4 is based on recent advances in machine learning, which allow technologies including large language models to be more cheaply and efficiently ‘fine-tuned’ on person-specific data. The P4, unlike the PPP, would be able to infer an individual patient’s preferences from material (e.g., prior treatment decisions) that is in fact specific to them. Thus, we argue, in addition to being potentially more accurate at the individual level than the previously proposed PPP, the predictions of a P4 would also more directly reflect each patient’s own reasons and values. In this article, we review recent discoveries in artificial intelligence research that suggest a P4 is technically feasible, and argue that, if it is developed and appropriately deployed, it should assuage some of the main autonomy-based concerns of critics of the original PPP. Animal models of human diseases are often used in biomedical research in place of human subjects. However, results obtained by animal models may fail to hold true for humans. One way of addressing this problem is to make animal models more similar to humans by placing human tissue into animal models, rendering them chimeric. Since technical and ethical limitations make neurological disorders difficult to study in humans, chimeric models with human neural tissue could help advance our understanding of neuropathophysiology. In (...) this article, we examine whether the introduction of human neural tissue and any consequent cognitive change is relevant to the way we ought to treat chimeras. We argue that changes in cognitive abilities are morally relevant to the extent that they increase the capacities that affect the moral status of any entity, including awareness, autonomy, and sociability. We posit that no being, regardless of species, should be treated in a way that is incommensurate with its moral status. Finally, we propose a framework that can be used to guide ethical assessment of research involving chimeras with advanced cognitive capacities. We advance this framework as a useful tool for bringing relevant considerations to the forefront for those considering the ethical merit of proposed chimeric research. The Durkheim Test. Remarks on Susan Leigh Star’s Boundary Objects. The article reconstructs Susan Leigh Star’s conceptual work on the notion of ‘boundary objects’. It traces the emergence of the concept, beginning with her PhD thesis and its publication as Regions of the Mind in 1989. ‘Boundary objects’ attempt to represent the distributed, multifold nature of scientific work and its mediations between different ‘social worlds’. Being addressed to several ‘communities of practice’, the term responded to questions from Distributed Artificial Intelligence (...) in Computer Science, Workplace Studies and Computer Supported Cooperative Work (CSCW), and microhistorical approaches inside the growing Science and Technology Studies. Yet the interdisciplinary character and interpretive flexibility of Star’s invention has rarely been noticed as a conceptual tool for media theory. Cloth, $39.95.Much ink has been spilled on the dispute between Ernst Cassirer and Martin Heidegger that took place in the Swiss resort town Davos in 1929—famous since Thomas Mann staged his Magic Mountain there—and which has since been referred to as the “Davos Dispute.” While the (...) debate itself and accounts accompanying it have been widely known to historians [End Page 508] of philosophy, Harvard historian Peter E. Gordon shows in this marvelous new book how memory has shaped the event into something much larger than anyone could have foreseen. It is, Gordon claims, the silent work of memory that has made this debate, rather innocuous at the time, into what can be retrospectively be considered the truly earth-shattering philosophical event of the twentieth century.In hindsight, the symbolism of the debate between a German who became a leading Nazi philosopher in 1933 and a Jew who, in the same year, was dismissed from his professorship and forced to flee Germany, could not be greater. It only grows if one connects the historic events with their respective philosophies: on the one side the liberal modern Cassirer, whose philosophy allegedly glorified the Weimar stalemate, and on the other side the radical Heidegger, whose call for “authentic existence” took on a bone-chilling reality as he embraced Hitler in 1933 as the führer to lead Germany back to its authentic roots.Two volumes—the 1999 collection, 70 Jahre Davoser Disputation, edited by E. Rudolph and D. Kaegi, and, in 2000, A Parting of the Ways by Michael Friedman—were the first to delve into the debate’s symbolic meaning. It was Friedman’s thesis that the fateful “parting of the ways” between Continental and Analytic philosophy could be traced to this meeting, which was also attended by Rudolf Carnap. Despite these studies, there has until now not been a comprehensive treatment that pulls together all threads into one narrative, covering the entire intellectual, historical, cultural, and philosophical landscape of the time. Gordon’s book achieves just that and will be, for years to come, the landmark work on this encounter.The book begins from the background in post-World War I philosophy in Germany, which was perceived, along with culture, as being in crisis (ch. 1), to “set the stage” on the “Magic Mountain” (ch. 2). It proceeds to summarize the individual lectures by Cassirer and Heidegger, giving a detailed interpretation of the protocol prepared by students of the interlocutors (chs. 3–4). The author then takes a deep breath in chapter 5 to place the debate in the larger context of Cassirer’s and Heidegger’s philosophies. But the story does not end there, as chapter 6 examines the relation between both thinkers after the debate. Finally, in chapter 7, Gordon assesses the newer debates concerning this dispute up to the debate between Bourdieu and Habermas concerning Heidegger’s Nazi involvement, though it does not re-engage with Friedman’s thesis, as might have been desired.Regarding his interpretation, Gordon juxtaposes Cassirer’s and Heidegger’s philosophies along the Kantian distinction between spontaneity and receptivity, where both paradigms make different claims on the “normative image of humanity” (5 ff.). Whether humanity is interpreted as spontaneous and world-creating, in optimistic Enlightenment terms (Cassirer), or as receptive and open to the world, i.e. as the “shepherd” rather than the “lord of being” (Heidegger), both are answering the fourth of Kant’s questions, “What is man?” While this distinction is useful for understanding the basic relation between them, it is also—as the author would concede—somewhat too coarse to do justice to their mutual differences and commonalities. Were one to assert a systematic critique, Gordon does not really make up his mind as to the true significance of these paradigms: are they mutually exclusive oppositions or, as he implies elsewhere, two alternative versions of philosophical modernism with a common ground? BackgroundOperation room (OR) planning is a complex process, especially in large hospitals with high rates of unplanned emergency procedures. Postponing elective surgery in order to provide capacity for emergency operations is inevitable at times. Elderly patients, residents of nursing homes, women, patients with low socioeconomic status and ethnic minorities are at risk for undertreatment in other contexts, as suggested by reports in the medical literature. We hypothesized that specific patient groups could be at higher risk for having their elective surgery (...) rescheduled for non-medical reasons.MethodsIn this single center, prospective observational trial, we analysed 2519 patients undergoing elective surgery from October 2018 to May 2019. A 14-item questionnaire was handed out to illicit patient details. Additional characteristics were collected using electronic patient records. Information on the timely performance of the scheduled surgery was obtained using the OR’s patient data management system. 6.45% of all planned procedures analysed were postponed. Association of specific variables with postponement rates were analysed using the Mann–Whitney U test and Fisher's exact test/χ2-test.ResultsSignificantly higher rates of postponing elective surgery were found in elderly patients. No significant differences in postponing rates were found for the variables gender, nationality (Germany, EU, non-EU), native language, professional medical background and level of education. Significantly lower rescheduling rates were found in patients with ties to hospital staff and in patients with a private health insurer.ConclusionsElderly patients, retirees and nursing home residents seem to be at higher risk for having their elective surgery rescheduled. However, owing to the study design, causality could not be proven. Our findings raise concern about possible undertreatment of these patient groups and provide data on short-term postponement of elective surgery.Trial registration DRKS00015836. 'Everyone knows what attention is’ according to William James. Much work on attention in psychology and neuroscience cites this famous phrase only to quickly dismiss it. But James is right about this: ‘attention’ was not introduced into psychology and neuroscience as a theoretical concept. I argue that we should therefore study attention with broadly the same methodology that David Marr has applied to the study of perception. By focusing more on Marr's Computational Level of analysis, we arrive at a unified (...) answer to the question of what attention is, what role it plays in the mind, and why organisms like us have that capacity. I propose a methodology for studying attention at Marr's Computational Level that optimizes in a three-dimensional space: it should capture core aspects of our first-person experience of attention, be explanatorily powerful in psychology and neuroscience, and fertile in an interdisciplinary context. I show how this methodology leads to what I call the priority structure account of attention. Attention is what organizes current information to make it more useful for the organism. We can identify it by four features. Attention, in this way, helps a cognitive system to integrate its informational state with its current motivational state. It has been argued that the explanation of self-control requires positing special motivational powers. Some think that we need will-power as an irreducible mental faculty; others that we need to think of the active self as a dedicated and depletable pool of psychic energy or – in today more respectable terminology – mental resources; finally, there is the idea that self-control requires postulating a deep division between reason and passion – a deliberative and an emotional motivational system. This essay argues (...) that no such special motivational powers are necessary. Yet, at the same time, self-control does powerfully illustrate the importance of a feature of the mind. What it illustrates, I argue, is the importance of the mental activity of attention in the control of all action. It is by appeal to this mental activity that we can dispense with special motivational powers. If we think of Humeanism as the view that there is fundamentally only one kind of motivational system and that all action is based in that system, then this essay contributes to a defense of Humeanism. On the other hand, the essay also shows that any model of agency in terms of only beliefs and desires, motivational and representational states, or preferences and credences is incomplete. A different conception of Humeanism as the view that every mental state is either motivational, representational, or a combination of them, is false. (shrink)

The normative force of evidence can seem puzzling. It seems that having conclusive evidence for a proposition does not, by itself, make it true that one ought to believe the proposition. But spelling out the condition that evidence must meet in order to provide us with genuine normative reasons for belief seems to lead us into a dilemma: the condition either fails to explain the normative significance of epistemic reasons or it renders the content of epistemic norms practical. The first (...) aim of this paper is to spell out this challenge for the normativity of evidence. I argue that the challenge rests on a plausible assumption about the conceptual connection between normative reasons and blameworthiness. The second aim of the paper is to show how we can meet the challenge by spelling out a concept of epistemic blameworthiness. Drawing on recent accounts of doxastic responsibility and epistemic blame, I suggest that the normativity of evidence is revealed in our practice of suspending epistemic trust in response to impaired epistemic relationships. Recognizing suspension of trust as a form of epistemic blame allows us to make sense of a purely epistemic kind of normativity the existence of which has recently been called into doubt by certain versions of pragmatism and instrumentalism. What is attention? How does attention shape consciousness? In an approach that engages with foundational topics in the philosophy of mind, the theory of action, psychology, and the neurosciences this book provides a unified and comprehensive answer to both questions. Sebastian Watzl shows that attention is a central structural feature of the mind. The first half of the book provides an account of the nature of attention. Attention is prioritizing, it consists in regulating priority structures. Attention is not another (...) element of the mind, but constituted by structures that organize, integrate, and coordinate the parts of our mind. Attention thus integrates the perceptual and intellectual, the cognitive and motivational, and the epistemic and practical. The second half of the book concerns the relationship between attention and consciousness. Watzl argues that attentional structure shapes consciousness into what is central and what is peripheral. The center-periphery structure of consciousness cannot be reduced to the structure of how the world appears to the subject. What it is like for us thus goes beyond the way the world appears to us. On this basis, a new view of consciousness is offered. In each conscious experience we actively take a stance on the world we appear to encounter. It is in this sense that our conscious experience is our subjective perspective. (shrink)

Many of our attitudes are non-culpable: there was nothing that we should have done to avoid holding them. I argue that we can still be blameworthy for non-culpable attitudes: they can impair our relationships in ways that make our full practice of apology and forgiveness intelligible. My argument poses a new challenge to indirect voluntarists, who attempt to reduce all responsibility for attitudes to responsibility for prior actions and omissions. Rationalists, who instead explain attitudinal responsibility by appeal to reasons-responsiveness, can (...) make sense of blameworthiness for non-culpable attitudes. In response, voluntarists could propose a revision of our actual practices. This would lead us into a quite different debate. (shrink)

Quasi-realists argue that meta-ethical expressivism is fully compatible with the central assumptions underlying ordinary moral practice. In a recent paper, Andy Egan has developed a vexing challenge for this project, arguing that expressivism is incompatible with central assumptions about error in moral judgments. In response, Simon Blackburn has argued that Egan's challenge fails, because Egan reads the expressivist as giving an account of moral error, rather than an account of judgments about moral error. ABSTRACT The difference between perception and cognition seems introspectively obvious in many cases. Perceiving and thinking have also been assigned quite different roles, in epistemology, in theories of reference and of mental content, in philosophy of psychology, and elsewhere. Yet what is the nature of the distinction? In what way, or ways, do perception and cognition differ? The paper reviews recent work on these questions. Four main respects in which perception and cognition have been held to differ are discussed. First, (...) their phenomenal character, such as the often-remarked vivacity or immediacy of perception. Second, the way in which they represent the world, e.g. the non-propositional nature of the contents, or non-discursive character of the vehicles, that have been held to characterise perceptual representation. Third, their place in cognitive architecture, i.e., roughly, in the information-flow of the mind, such as their alleged (non-)modularity. Fourth, their mind-world relations, e.g. the way in which perceptions seem to be tightly causally linked with distal or proximal stimuli. This paper argues for the normative significance of attention. Attention plays an important role when describing an individual’s mind and agency, and in explaining many central facts about that individual. In addition, many in the public want answers and guidance with regard to normative questions about attention. Given that attention is both descriptively central and the public cares about normative guidance with regard to it, attention should be central also in normative philosophy. We need an ethics of attention: a field (...) of study of which normative pressures, if any, govern attention. Like the ethics of belief, the ethics of attention will connect those normative questions to issues regarding the nature of attention, i.e. to what is subject to such normative pressures. Philosophers should develop an ethics of attention that helps to provide normative guidance and that is commensurate in its richness to the descriptive significance of attention. The second half of the paper sketches a framework that may help us to get started at developing the ethics of attention. (shrink)

This paper defends and develops the structuring account of conscious attention: attention is the conscious mental process of structuring one’s stream of consciousness so that some parts of it are more central than others. In the first part of the paper, I motivate the structuring account. Drawing on a variety of resources I argue that the phenomenology of attention cannot be fully captured in terms of how the world appears to the subject, as well as against an atomistic conception of (...) attention. In the second part of the paper, I show how the structuring account can be made precise: attention causes and causally sustains phenomenal relations to hold between the parts of the stream of consciousness; most importantly the relation of one part being peripheral to another. I end by pointing out consequences for both the scientific study of attention as well as for several areas of central philosophical interest. (shrink)

Cultural rights constitute one of the most exciting new frontiers of human rights research and practice. Cultural rights are also the ultimate law and humanities topic. These are good enough reasons for making cultural rights the main focus of a book. But there are other reasons, too. Cultural rights are both transformative and empowering rights. They enable people to aspire to a better future for themselves, their families, and the society in which they live, and they play a key role (...) in realizing all other human rights. Furthermore, they provide a much-needed discourse or common forum in which we can explore, negotiate, and come to new cross-cultural understandings. All of this is supremely important today, at a time when respect for cultural diversity is a key concern worldwide, and when migration and advances in technology are increasing the level of cultural exchange but also fostering cultural clashes and incompatibilities previously masked by distance. What is attention? Attention is often seen as a subject matter for the hard sciences of cognitive and brain processes, and is understood in terms of sub-personal mechanisms and processes. Correspondingly, there still is a stark contrast between the central role attention plays for the empirical investigation of the mind in psychology and the neurosciences, and its relative neglect in philosophy. Yet, over the past years, several philosophers have challenged the standard conception. A number of interesting philosophical questions concerning the (...) nature of attention arise. This article provides an introduction to contemporary debates concerning these questions. In particular, it discusses the question of how the pre-theoretic conception of attention might be reconciled with a scientific conception, arguments that provide support for an anti-reductivist theory of attention, and sketches several recent anti-reductivist theories and their inter-relations. What is the philosophical significance of attention? The present article provides an overview of recent debates surrounding the connections between attention and other topics of philosophical interest. In particular, it discusses the interplay between attention and consciousness, attention and agency, and attention and reference. The article outlines the questions and contemporary positions concerning how attention shapes the phenomenal character of experience, whether it is necessary or sufficient for consciousness, and whether it plays a special role in the best philosophical theories (...) of action or conceptual reference. Various interdependencies between the answers to these questions are indicated, as well as how these answers might depend on the metaphysics of attention. In “Risk Based Passenger Screening in Aviation Security: Implications and Variants of a New Paradigm”, Sebastian Weydner-Volkmann describes the current paradigm shift from ‘traditional’ forms of screening to ‘risk based passenger screening’ (RBS) in aviation security. This paradigm shift is put in the context of the wider historical development of risk management approaches. Through a discussion of Michel Foucault, Herfried Münkler and Ulrich Beck, Weydner-Volkmann analyses the shortcomings of such approaches in public security policies, which become especially evident in (...) the aviation security context. As he shows, the turn towards methods of RBS can be seen as an attempt to address a trade-off ‘trilemma’ between the effective provision of security, the implied costs for industry and passengers, and the ethical, legal and societal implications of the screening procedures. In order to analyse foreseeable outcomes of embracing RBS, he differentiates three prototypical variants of the new paradigm on the basis of their main referent and rationale. For each variant, he then subsequently assesses the implications for the ‘trilemma’, after having unveiled the criteria of analysis that will necessarily have to be followed within a serious appraisal of RBS methods. This dissertation investigates the nature, the phenomenal character and the philosophical significance of attention. According to its central thesis, attention is the ongoing mental activity of structuring the stream of consciousness or phenomenal field. The dissertation connects the scientific study of attention in psychology and the neurosciences with central discussions in the philosophy of mind. Once we get clear on the nature and the phenomenal character of attention, we can make progress toward understanding foundational issues concerning the nature and the (...) structure of conscious mentality itself. We understand better how consciousness is connected to self-awareness and to agency, and we get a better grip on the nature of perceptual experience, the unity of consciousness, and its subjective character. The dissertation also aims at showing that the current empirical investigation of attention should be complemented with work at the level of generality that a philosophical analysis can provide; it shows how such an analysis is relevant for the scientific study of attention by providing a new conceptual framework and suggesting several new areas of research. We argue that Wittgenstein’s philosophical perspective on Gödel’s most famous theorem is even more radical than has commonly been assumed. Wittgenstein shows in detail that there is no way that the Gödelian construct of a string of signs could be assigned a useful function within (ordinary) mathematics. — The focus is on Appendix III to Part I of Remarks on the Foundations of Mathematics. Perceptual illusions have often served as an important tool in the study of perceptual experience. In this paper I argue that a recently discovered set of visual illusions sheds new light on the nature of time consciousness. I suggest the study of these silencing illusions as a tool kit for any philosopher interested in the experience of time and show how to better understand time consciousness by combining detailed empirical investigations with a detailed philosophical analysis. In addition, and more specifically, (...) I argue against an initially plausible range of views that assume a close match between the temporal content of visual experience and the temporal layout of experience itself. Against such a widely held structural matching thesis I argue that which temporal changes we are experiencing bears no close relation to how our experience itself is changing over time. Explanations of the silencing illusions that are compatible with the structural matching thesis fail. (shrink)

Could the organization of consciousness be the key to understanding its unity? This paper considers how the attentional organization of consciousness into centre and periphery bears on the phenomenal unity of consciousness. Two ideas are discussed: according to the first, the attentional organization of consciousness shows that phenomenal holism is true. I argue that the argument from attentional organization to phenomenal holism remains inconclusive. According to the second idea, attentional organization provides a principle of unity for conscious experience, i.e. it (...) is a relationship between the phenomenal parts that occurs in the real definition or essence of consciousness. Conscious experience provides subjects with a subjective perspective, or point of view, because its various parts are structured by attention into what is more central and what is more peripheral. I argue first that attention is a (maybe the) paradigmatic case of an object-directed, non-propositional intentional mental episode. In addition attention cannot be reduced to any other (propositional or non-propositional) mental episodes. Yet, second, attention is not a non-propositional mental attitude. It might appear puzzling how one could hold both of these claims. I show how to combine them, and how that combination shows how propositionality and non-propositionality can co-exist in a mental life. Recent psychological research shows that attention affects appearances. An “attended item looks bigger, faster, earlier, more saturated, stripier.” (Block 2010, p. 41). What is the significance of these findings? Ned Block has argued that they undermine representationism, roughly the view that the phenomenal character of perception is determined by its representational content. My first goal in this paper is to show that Block’s argument has the structure of a Problem of Arbitrary Phenomenal Variation and that it improves on other instances (...) of arguments of the same form along several dimensions (most prominently, these are arguments based on the possibility of spectral inversion). My second goal is to consider responses to Block’s version of the arbitrariness problem. I will show that most of them have serious drawbacks. Overall, the best view is to accept that attention may distort perception, sacrificing veridicality for usability. I end my discussion by showing how to develop that view. (shrink)

Discussions regarding which norms, if any, govern our practices of forming, maintaining and relinquishing beliefs have come to be collected under the label “The ethics of belief”. Included in the ethics of belief are debates about how those normative issues relate to the nature of belief, whether belief formation is, for example, ever voluntary. The present talk concerns an analogous set of questions regarding our practices of attention. “The ethics of attention” thus concerns the discussion of which norms, if any, (...) govern our practices of attention: what norms govern what we should attend to, how we should engage our capacity for attention, when we should begin and when we should stop to pay attention to something? Like the ethics of belief, the ethics of attention will connect those normative questions to issues regarding the nature of attention, what may or may not be subject to such normative pressures. Compared with rich, complex, and systematic investigation of the ethics of belief, the study of the ethics of attention is more or less undeveloped. This paper aims to begin to change that. Specifically, it shows that attention is an appropriate target for serious normative investigation and then classifies potential norms of attention along three dimensions: whether they are manner or object based, instrumental or non-instrumental, and whether its source is moral, prudential or epistemic. -/- [EDIT. 10.
