ObjectivesDepression and anxiety persist in cancer patients, creating an additional burden during treatment and making it more challenging in terms of management and control. Studies on the prevalence of depression and anxiety among cancer patients in the Middle East are limited and include many limitations such as their small sample sizes and restriction to a specific type of cancer in specific clinical settings. This study aimed to describe the prevalence and risk factors of depression and anxiety among cancer patients in (...) the inpatient and outpatient settings.Materials and MethodsA total of 1,011 patients formed the study sample. Patients’ psychological status was assessed using the Hospital Anxiety and Depression Scale, the Patient Health Questionnaire, and the Generalized Anxiety Disorder 7-item scale. The prevalence rate of depressive and anxious symptomatology was estimated by dividing the number of patients who exceeded the borderline score: 10 or more for each subscale of the HADS scale, 15 or more for the GAD-7 scale, and 15 or more in the PHQ-9 by the total number of the patients. Risk factors were identified using logistic regression.ResultsThe prevalence of depressive and anxious symptomatology among all patients was 23.4% and 19.1–19.9%, respectively. Depressive symptomatology was more prevalent across patients who were hospitalized compared with patients in the outpatient setting. Similarly, anxious symptomatology was more prevalent in the inpatient setting. In the inpatient setting, depressive symptomatology was more prevalent among patients with bladder cancer, while severe anxious symptomatology was more prevalent across patients with lung cancer. In the outpatient setting, depressive and anxious symptomatology was more prevalent among breast and prostate cancer patients, respectively. Despite that, around 42.7% and 24.8% of the patients, respectively, reported that they feel anxious and depressed, and only 15.5% of them were using medications to manage their conditions.ConclusionOur study findings demonstrated a higher prevalence of depressive and anxious symptomatology in the inpatient setting and advanced disease stages. In addition, the underutilization of antidepressant therapy was observed. There is a need to consider mental disorders as part of the treatment protocol for cancer patients. Enhanced clinical monitoring and treatment of depression and anxiety of cancer patients are required. (shrink)

BackgroundEthical care provided by nurses to earthquake victims is one of the main subjects in nursing profession.ObjectivesGiven the information gap in this field, the present study is an attempt to explore the nurses’ experience of ethical care provided to victims of an earthquake.Research design and methodA hermeneutic phenomenological study was performed. The participants were 16 nurses involved in providing care to the injured in Kermanshah earthquake, Iran. They were selected using purposeful sampling, and in-depth and semi-structured interviews were carried out. (...) The transcribed interviews were analyzed based on the hermeneutic approach using the analysis method proposed by Diekelmann et al.Ethical considerationsThe study was approved by the Research Council and Ethics Committee of Urmia University of Medical Sciences, Iran.FindingsData analyses revealed four themes and 10 sub-themes that illustrated nurses’ experience of ethical care during earthquake. The themes were (1) Respecting humanistic values (sacrifice, stepping beyond task description, and voluntary work), (2) Commitment to ethics (honesty, confidentiality, and trustworthiness), (3) Respecting dignity of victims (respecting cultural values, maintaining privacy, having humanistic perspective, and effective communication), and (4) Spiritual support (helping patients to do religious rituals Psychological support).ConclusionThe results showed the nurses’ experience with providing care to earthquake victims. The findings underlined ethics and ethical values in providing nursing care during disasters. It is suggested that special courses on the importance of nursing ethics in critical situations be incorporated into nursing curriculums and in-service educations. (shrink)

The study focuses on the emotional-motivational experiences of Bedouin-Arab beginning teachers during the induction period, from the perspective of Self-Determination Theory. A phenomenological study was employed. Seventy-four teachers participated, 62 of whom completed open questionnaires, while semi-structured interviews were conducted with 12 other participants. The findings indicate that the beginning teachers reported experiences of coercion, exploitation, and gender-based discrimination. They also experienced a judgmental attitude, lack of assistance, and difficulties with students, and their sense of relatedness to the school is (...) impaired due to cultural factors. As a result, they expressed controlled motivation, a sense of burnout, stress, impaired well-being and disengagement in school. They also suppressed their students' autonomy. At the same time, the findings also show that when the teachers experience a sense of need satisfaction, they integrate well into the school. These findings indicate the necessity for establishing a need-supportive school environment for beginning teachers during their induction period. (shrink)

The induction period is considered one of the most difficult in a teacher’s career. In Israel, support systems for beginning teachers include a learning community and a mentoring process, over a 2-year period. The study was based on self-determination theory and examined how support for BTs’ psychological needs and exploration from the LC facilitator and teacher-mentor contributed to their functioning. The study was conducted over 2 years during which BTs participated in LCs and were accompanied by a teacher-mentor. Questionnaires were (...) administered to Bedouin-Arab and Jewish teachers, 308 interns in the first year, and 205 new teachers in the second. Results of SEM analysis indicated that supporting BTs’ needs and exploration by LC facilitators in both years predicted their autonomous motivation, which in turn predicted positive feelings and satisfaction in the LC. The LC facilitators’ support also predicted the BTs’ autonomous motivation in teaching, which was also predicted by the teacher-mentors’ support. This in turn positively predicted the BTs’ sense of competence and self-actualization, and negatively predicted burnout. In both years, autonomous motivation mediated the association between the support the BTs received, and teacher outcomes in the LC and school. No significant differences were found between the two cultures. The effects found in the second year are above and beyond the effects of support from the LC facilitators and teacher-mentors in the first year. The study indicates the importance of combining multiple induction programs over time and highlights the importance of supporting BTs’ needs during the induction period. (shrink)

Buildings energy consumption is growing gradually and put away around 40% of total energy use. Predicting heating and cooling loads of a building in the initial phase of the design to find out optimal solutions amongst different designs is very important, as ell as in the operating phase after the building has been finished for efficient energy. In this study, an artificial neural network model was designed and developed for predicting heating and cooling loads of a building based on a (...) dataset for building energy performance. The main factors for input variables are: relative compactness, roof area, overall height, surface area, glazing are a, wall area, glazing area distribution of a building, orientation, and the output variables: heating and cooling loads of the building. The dataset used for training are the data published in the literature for various 768 residential buildings. The model was trained and validated, most important factors affecting heating load and cooling load are identified, and the accuracy for the validation was 99.60%. (shrink)

Medical research must involve the participation of human subjects. Knowledge of patients' perspectives and concerns with their involvement in research would enhance recruitment efforts, improve the informed consent process, and enhance the overall trust between patients and investigators. Several studies have examined the views of patients from Western countries. There is limited empirical research involving the perspectives of individuals from developing countries. The purpose of this study is to examine the attitudes of Egyptian individuals toward medical research. This paper addresses the impact of the unethical business conduct of a few individuals that shook the financial market in 1986. Specifically, in the study undertaken for this paper, the wealth status of the shareholders of securities firms was examined in relation to the public disclosure of the insider-trading scandals involving Dennis Levine, Ivan Boesky, and their confederates. It was hypothesized that the expected market-adjusted stock returns for the securities firms would be negative as a result of the scandals. The (...) findings of the study supported the hypothesis. Alfred I. Tauber (ed.), Organism and the Origins of Self. Dordrecht, Netherlands: Kluwer Academic Publishers, 1991. xix + 384 pp., US$ 110.00 (US$ 25.00 paperback). This is a fascinating book based on a 1990 symposium at Boston University. It promises to change the way one conceives of the organism. The authors start from different specializations but provide a most tantalizing feast of ideas. Richard Lewontin commences the book with a strange foreword. Lewontin submits that the concern with the "self and (...) private identity in modern biology is ... only one manifestation of the ideology of individualism that is a shibboleth of our particular [bourgeois] form of social organization" (p. xix). If this is the case, the reader should stop reading the book and ask for her money back. But I advise patience. Tauber's introduction is a mixture of the history of the medical notion of disease and the advocacy of a dialectical concept of the self as the interpenetration of part and whole. Tauber highlights Elie Metchnikoff's appealing idea of phagocytosis as just one form of the active organism striving to attain harmony among competing cells. (shrink)

According to Tocqueville, the freedom of the press, which he treats as an extension of the freedom of speech, is a primary constituent element of liberty. Tocqueville treats the freedom of the press in relation to and as an extension of the right to assemble and govern one’s own affairs, both of which he argues are essential to preserving liberty in a free society. Although scholars acknowledge the importance of civil associations to liberty in Tocqueville’s political thought, they routinely ignore (...) the importance he places on the freedom of the press and speech. His reflections on the importance of the free press and speech may help to shed light on the dangers of recent attempts to censor the press and speech. (shrink)

Based on case studies of religious Muslim and Christian family firms operating in a religiously diverse country, we explain the multiplicity of family, business, religion, and community logics in the family firm. In particular, we give attention to the religion logic and how it interacts with other logics when family firms are considering ethical issues. We show that religion has a rule-based approach in Muslim family firms and a principle-based approach in Christian family firms. We also draw attention to the (...) fluidity characteristic of the religion logic, through which family firms interpret the role of religion among other logics in influencing ethical decisions. Our study advances institutional logics literature in highlighting the plurality between and within logics in family firms, and contributes to the growing recognition of the influence of religious beliefs on the ethical behaviors of family firms. Background Paediatric clinical research faces unique challenges that compromise optimal recruitment of children into clinical trials. A main barrier to enrolment of children is parental misconceptions about the research process. In developing countries, there is a knowledge gap regarding parental perceptions of and attitudes towards their children's participation in clinical trials. Objective To explore such perceptions and attitudes in Lebanese parents. Study design 33 in-depth interviews were conducted with parents with and without previous research experience. Interviews were tape-recorded, transcribed in (...) colloquial Arabic, and later subjected to thematic analysis. Results Benefit/risk ratio assessment was a major determinant of parental consent. Fear of adverse events or painful procedures in research was a recurring theme in most interviews. Whereas perception of direct benefit to the child, trust in the physician or institution, financial gains or having a positive previous experience in research facilitated consent, a complex informed consent form and misunderstanding of the term ‘randomisation’ hindered parental approval of participation. Conclusion Lebanese parents have perceptions of and attitudes towards children's participation in clinical trials that are similar to those reported from the industrialised world. Improving communication with parents and building trust between researchers and parents is important for successful recruitment. Investigators planning to conduct paediatric trials in developing countries need to simplify consent forms and devise new ways to explain randomisation. (shrink)

BackgroundStudies have shown that research participants fail to appreciate the difference between research and medical care, labeling such phenomenon as a "therapeutic misconception" (TM). Since research activity involving human participants is increasing in the Middle East, qualitative research investigating aspects of TM is warranted. Our objective was to assess for the existence of therapeutic misconception amongst Egyptians.MethodsStudy Tool: We developed a semi-structured interview guide to elicit the knowledge, attitudes, and perspectives of Egyptians regarding medical research.Setting: We recruited individuals from the (...) outpatient settings (public and private) at Ain Shams University in Cairo, Egypt.Analysis: Interviews were taped, transcribed, and translated. We analyzed the content of the transcribed text to identify the presence of a TM, defined in one of two ways: TM1 = inaccurate beliefs about how individualized care can be compromised by the procedures in the research and TM2 = inaccurate appraisal of benefit obtained from the research study.ResultsOur findings showed that a majority of participants (11/15) expressed inaccurate beliefs regarding the degree with which individualized care will be maintained in the research setting (TM1) and a smaller number of participants (5/15) manifested an unreasonable belief in the likelihood of benefits to be obtained from a research study (TM2). A total of 12 of the 15 participants were judged to have expressed a TM on either one of these bases.ConclusionThe presence of TM is not uncommon amongst Egyptian individuals. We recommend further qualitative studies investigating aspects of TM involving a larger sample size distinguished by different types of illnesses and socio-economic variables, as well as those who have and have not participated in clinical research. The ArgumentThe remarkable progress in modern genetic technology enables the identification of genes causing devastating diseases and thereby the development of tools for prenatal diagnosis and carrier detection. To implement the results of genetic research in traditional societies, where genetic diseases are more prevalent due to inbreeding, necessitates a culturally appropriate approach that also promotes traditional and societal values important to the relevant community. This paper presents our experience with implementing the results of modern genetic research among the traditional community (...) of the Negev Bedouin of Israel. Although the benefit of using those results for the prevention of genetic diseases seems obvious, successful implementation relies on a carefully designed educational program aimed at changing culturally related attitudes and perceptions. Such a program should attend to the needs of the community and be sensitive to its traditional values. (shrink)

Ovaj rad odgovara na argument modalnog kolapsa protiv božanske jednostavnosti ili klasičnog teizma koji nude neoklasični ili kompleksni teisti. Argument modalnog kolapsa tvrdi da ako je Bog apsolutno jednostavan i apsolutno neophodan, tada je Božji čin stvaranja apsolutno neophodan, pa je stoga postojanje stvorenog svijeta također apsolutno neophodno. To znači da se Bog i njegova kreacija urušavaju u jednu modalnu kategoriju apsolutne nužnosti bez ikakvih kontingentnih bića. Moj odgovor je utemeljen na islamskoj neoplatonstičkoj filozofiji Ibn Sine i islamskoj tradiciji. Nudim (...) četiri argumenta koji dopuštaju muslimanskom neoplatoničaru da apsorbira modalni kolaps u modalitetu mogućih svjetova koji istovremeno može negira modalni kolaps unutar avicenovskog modaliteta: Prvo, prigovor modalnog kolapsa temelji se na okviru mogućih svjetova čiji je pojam nužnosti preširok; ovaj okvir ne uspijeva napraviti razliku između Boga kao ontološki nužnog po sebi, stvorenog bića kao nužno ovisnog o drugome i puke logičke nužnosti, kao što to čine Ibn Sina i islamski mislioci. Drugo, argumenti modalnog kolapsa samo pokazuju da je kreacija neophodna kroz drugost, ali ne uspijevaju dokazati da kreacija ima ontološku nužnost ili aseity––što se odnosi samo na Boga; prema tome, ne dolazi do posljedičnog modalnog kolapsa kada nečiji modalitet prepozna kreaciju kao "ovisno nužno biće" unatoč tome što je modalno nužno. Treće, islamski filozofi imaju nelibertarijanski pojam Božje volje i slobode koji je imun na prigovore modalnog kolapsa. Konačno, tvrdim da svi klasični i neoklasični teisti moraju prihvatiti modalno nužnu kreaciju jer libertarijanski modeli Božje volje za sobom povlače neuzrokovane i primitivno kontingentne učinke. El autor analiza tanto las causas religiosas como las debidas a factores históricos y sociales: cambio en los criterios del reparto del botín de guerra, en el trato y uso de las prisioneras de guerra, introducción del concepto de umm al-walad, etc. Igualmente, se estudia la repercusión social que tuvieron estos cambios para las esclavas (...) y para las familias a las que pertenecían. Este artículo se ocupa de los reyes tribales en la Arabia pre-islámica. Estos reyes, eran en realidad jefes tribales que llevaban el título de malik y se tocaban con coronas. Algunos derivaban su poder del emperador sasánida, que eran quien les concedía las coronas. Su autoridad era principalmente local y limitada al territorio concreto de sus propias tribus; en algunos casos, a través de una confederación tribal, podía extenderse a otros territorios. Apoyados por guarniciones de la caballería persa, los reyes (...) podían ejercer su poder sobre el territorio y la población, lo que se traducía en un tributo anual pagado por los habitantes de las áreas bajo control. Asimismo, los reyes controlaban los mercados estacionales que se celebraran en su territorio y las rutas comerciales que lo atravesaban. A cambio, eran responsables de las vidas de sus contríbulos y de la seguridad de sus propiedades. (shrink)

The denomination of personal being as lord of time signifies that the essence of human being appears in the condition of essential anteposition related to time. According to it, the human being disposes of time in presential articulation, which depends on the anthropological real distinction of essentia and esse. This study includes the metaphysic examination or pure values of before and after and it tries to solve the difficulties in the order of pure thinking of time.

This paper analyses Edith Stein's thought in order to find out, particularly in the investigation the philosopher did about Thomas Aquinas, the metaphysical assumptions hidden in the phenomeno-logy. It tries to clarify in which way the steinian "realism of essence", based on a certain logical atomism, gives up the idealistic postulates of Husserl's philosophy.

Leibnizian senses of temporality are analized, in order to assess that Leibniz’s formal and a priori notion of time is based on his own modal metaphysics. In this way, we can say that time constitutes the notion that allows the modalities articulation (Possibility-Necessity-Contingency) as well as the role of modal concepts allows to control the time.

The mental limit may be described as formal fullness of time in accordance with its conditional character in relation with intelligible essence. The metaphysic knowledge includes a methodic exclusion confining time under thematic position in order to its abandonment. The abandonment of formal fullness of time notices being. As a conditional antecedent of thought object, the limit is behind practical action and it is trespassed in it.

RESUMENExamina el empleo dialéctico de las que se consideran principales modalidades del pensamiento en relación con el método: la articulación del tiempo, la reflexión negativa y el pensar como ejercicio en dependencia de la libertad trascendental. Concluye que Hegel establece una fusión entre tales direcciones principales del método con vistas al establecimiento de su comienzo absoluto.
