Although many physicians have been using the internet for both clinical and social purposes for years, recently concerns have been raised regarding blurred boundaries of the profession as a whole. In both the news media and medical literature, physicians have noted there are unanswered questions in these areas, and that professional self-regulation is needed. This report discusses the ethical implications of physicians’ nonclinical use of the internet, including the use of social networking sites, blogs, and other means to post content (...) online. It does not address the clinical use of the internet, such as telemedicine, e-prescribing, online clinical consultations, health-related websites, use of electronic media for clinical collaboration, and e-mailing patients (some of which are already covered in the AMA’s Code of Medical Ethics). Free markets alone do not work effectively to solve certain kinds of human problems, such as education, old age care, or disaster relief. Nor have markets ever been the sole solution to the psychological challenges of death, suffering, or injustice. Instead, we find a major role for the non-market institutions of society - the family, the state, and social institutions. The first in-depth anthropological study of charities in contemporary Chinese societies, this book focuses on the unique ways that religious groups (...) have helped to solve the problems of social well-being. Using comparative case studies in China, Taiwan and Malaysia during the 1980s and onwards, it identifies new forms of religious philanthropy as well as new ideas of social 'good', including different forms of political merit-making, new forms of civic selfhood, and the rise of innovative social forms, including increased leadership by women. The book finally argues that the spread of these ideas is an incomplete process, with many alternative notions of goodness continuing to be influential. (shrink)

Recent empirical work has offered strong support for ‘biased pluralism’ and ‘economic elite’ accounts of political power in the United States, according a central role to ‘business interest groups’ as a mechanism through which corporate influence is exerted. Here, we propose an additional channel of influence for corporate interests: the ‘policy-planning network,’ consisting of corporate-dominated foundations, think tanks, and elite policy-discussion groups. To evaluate this assertion, we consider one key policy-discussion group, the Council on Foreign Relations. We first briefly review (...) the origins of this organization and then review earlier findings on its influence. We then code CFR policy preferences on 295 foreign policy issues during the 1981–2002 period. In logistic regression analyses, we find that the preferences of more affluent citizens and the CFR were positive, statistically significant predictors of foreign policy outcomes while business interest group preferences were not. These findings are discussed with a consideration of the patterns of CFR ‘successes’ and ‘failures’ for the issues in our dataset. Although a full demonstration of a causal connection between corporate interests, the policy-planning network and policy outcomes requires further research, we conclude that this has been shown to be a plausible mechanism through which corporate interests are represented and that ‘biased pluralism’ researchers should include it in their future investigations. Background The use of lengthy, detailed, and complex informed consent forms is of paramount concern in biomedical research as it may not truly promote the rights and interests of research participants. The extent of information in ICFs has been the subject of debates for decades; however, no clear guidance is given. Thus, the objective of this study was to determine the perspectives of research participants about the type and extent of information they need when they are invited to participate in (...) biomedical research. Methods This multi-center, cross-sectional, descriptive survey was conducted at 54 study sites in seven Asia-Pacific countries. A modified Likert-scale questionnaire was used to determine the importance of each element in the ICF among research participants of a biomedical study, with an anchored rating scale from 1 to 5. Results Of the 2484 questionnaires distributed, 2113 were returned. The majority of respondents considered most elements required in the ICF to be ‘moderately important’ to ‘very important’ for their decision making. Major foreseeable risk, direct benefit, and common adverse effects of the intervention were considered to be of most concerned elements in the ICF. Conclusions Research participants would like to be informed of the ICF elements required by ethical guidelines and regulations; however, the importance of each element varied, e.g., risk and benefit associated with research participants were considered to be more important than the general nature or technical details of research. Using a participant-oriented approach by providing more details of the participant-interested elements while avoiding unnecessarily lengthy details of other less important elements would enhance the quality of the ICF. This paper proposes a novel answer to the question of what attitude agents should adopt when they receive misleading higher-order evidence that avoids the drawbacks of existing views. The answer builds on the independently motivated observation that there is a difference between attitudes that agents form as conclusions of their reasoning, called terminal attitudes, and attitudes that are formed in a transitional manner in the process of reasoning, called transitional attitudes. Terminal and transitional attitudes differ both in their descriptive and (...) in their normative properties. When an agent receives higher-order evidence that they might have reasoned incorrectly to a belief or credence towards p, then their attitude towards p is no longer justified as a terminal attitude towards p, but it can still be justified as a transitional attitude. This view, which I call the unmooring view, allows us to capture the rational impact of misleading higher-order evidence in a way that integrates smoothly with a natural picture of epistemic justification and the dynamics of deliberation. Sorensen offers the following definition of a ‘knowledge-lie’: ‘An assertion that p is a knowledge-lie exactly if intended to prevent the addressee from knowing that p is untrue but is not intended to deceive the addressee into believing p.’ According to Sorensen, knowledge-lies are not meant to deceive their addressee, and this fact is supposed to make them less bad than ordinary lies. I will argue that standard cases of knowledge-lies, including almost all the cases Sorensen considers, do in fact (...) involve deception, contrary to what Sorensen claims. And while there are cases of non-deceptive knowledge-lies, such cases are deviant, either because it is only incidental that the knowledge-preventing assertion is a lie, or because it is only incidental that the lie doesn't deceive. Here's an example of a knowledge-lie: Dr Head is considering firing Dr Appendix because of his weak research. But he doesn't want to fire Appendix unless he knows that Appendix's research isn't good, and so he consults Dr Heart. Pancreatitis is a complex, progressively destructive inflammatory disorder. Alcohol was long thought to be the primary causative agent, but genetic contributions have been of interest since the discovery that rare PRSS1, CFTR and SPINK1 variants were associated with pancreatitis risk. We now report two associations at genome-wide significance identified and replicated at PRSS1-PRSS2 and X-linked CLDN2 through a two-stage genome-wide study. The PRSS1 variant likely affects disease susceptibility by altering expression of the primary trypsinogen gene. The CLDN2 risk allele is (...) associated with atypical localization of claudin-2 in pancreatic acinar cells. The homozygous CLDN2 genotype confers the greatest risk, and its alleles interact with alcohol consumption to amplify risk. Single-sided deafness (SSD) or profound unilateral hearing loss obligates the only serviceable ear to capture all acoustic information. This loss of binaural function taxes cognitive resources for accurate listening performance, especially under adverse environments or challenging tasks. We hypothesized that adults with SSD would manifest both functional and structural brain plasticity compared to controls with normal binaural hearing. We evaluated functional alterations using magnetoencephalographic imaging (MEGI) of brain activation during performance of a moderately difficult auditory syllable sequence reproduction task and (...) assessed structural integrity using diffusion tensor magnetic resonance imaging (DTI). MEGI showed the SSD cohort to have increased induced oscillations in the theta band over the left superior temporal cortex and decreased induced gamma band oscillations over the frontal and parietal cortices between 175-475 ms following stimulus onset. DTI showed the SSD cohort to have extensive fractional anisotropy reduction in both auditory and non-auditory tracts and regions. Overlaying functional and structural changes revealed by the two imaging techniques demonstrated close registration of cortical areas and white matter tracts that expressed brain plasticity. Hence, complete loss of input from one ear in adulthood triggers both functional and structural alterations to dorsal temporal and frontal-parietal areas. (shrink)

Theta-burst stimulation is a modification of TMS with similar efficacy and shorter session duration. The geriatric population has many comorbidities and a high prevalence of depression, but few clinical trials are conducted specifically for this age group. TBS could be an option in this population, offering the advantages of few side effects and no pharmacological interactions. When we’re inquiring to find out whether p is true, knowing that we’ll get better evidence in the future seems like a good reason to suspend judgment about p now. But, as Matt McGrath has recently argued, this natural thought is in deep tension with traditional accounts of justification. On traditional views of justification, which doxastic attitude you are justified in having now depends on your current evidence, not on what you might learn later. McGrath proposes to resolve this tension (...) by distinguishing between different ways of having a neutral attitude. I argue that McGrath’s account is unable to account for the full range of cases in which an agnostic attitude is warranted. We can remedy this by pairing his account with my theory of transitional and terminal attitudes, which claims that attitudes are justified in different ways depending on whether they are formed in intermediate stages of deliberation or as conclusions of deliberation. I compare my view with an alternative, more parsimonious one, according to which deliberation itself is a source of new evidence. I argue that this alternative proposal is faced with a dilemma: it either generates a vicious regress, or it fails to capture the relevant cases. (shrink)

In lieu of an abstract, here is a brief excerpt of the content:Julia Annas ACTION AND CHARACTER IN DOSTOYEVSKY'S NOTES FROM UNDERGROUND Notes from Underground was written with a specific purpose in mind: to answer Chernyshevsky's novel What Is to Be Done?1 And many features of Dostoyevsky's work can only be understood when we bear in mind its specifically Russian setting. The narrator is a romantic idealist of the forties transformed into something rather different by 1864, and no doubt we (...) lose much if we do not bear in mind that Dostoyevsky is looking back across the gulf of imprisonment and suffering at his own idealistic youth.2 But the intense and radically peculiar nature of Dostoyevsky's writing takes us to a level of the work which is accessible to those without knowledge of the local conditions of the work's production. As Mochulsky says, "the author steadily emerges beyond the confines of the Russian intellectual... the underground man's paradoxes are not the whims of some half-mad eccentric, but a new revelation of man about man." The book is "the philosophical preface to the cycle of the great novels" (p. 245). Even though Dostoyevsky's passionate and extreme temperament made him quite incapable of constructing a piece of precise philosophical argument, there is much of genuine philosophical interest in part I. It is not just a particular moral and political theory, like Chernyshevsky's, which is to be discredited, but something deeper, the presuppositions of a whole type of moral theory. In this article I shall examine the implications of what Dostoyevsky says for the philosophy of action and thence for ethics. I shall argue that he challenges a very basic model of human action, which is both intuitively plausible and basic to many moral theories. I shall also argue that as the work stands, there is a lack of continuity between parts I and II on the philosophical as well as on the literary level. In concentrating on the consequences for moral theory, I shall be ignoring the social and political aspects of the work. It may well be urged that such a division of the moral from the political is unrealistic in treating 257 258Philosophy and Literature a Russian writer. In defense I can only say that this narrowing of focus brings out in a sharper and more tractable way the philosophical issue which is my main concern. I Tell me, who was it who first declared, proclaiming it to the whole world, that a man does evil only because he does not know his real interests, and if he is enlightened and has his eyes opened to his own best and normal interests, man will cease to do evil and at once become virtuous and noble, because when he is enlightened and understands what will really benefit him he will see his own best interest in virtue, and since it is well known that no man can knowingly act against his best interests, consequently he will inevitably, so to speak, begin to do good. What exactly is the target here of the Underground Man?3 Every moral theory presupposes some theory of human action, although this may not be explicit; since moral philosophy is concerned with human actions, it must presuppose some account of what it is for a human action to be performed. One very influential tradition thinks of the paradigm of action as essentially aimed at some good: "Every art and every inquiry, and similarly every action and pursuit, is thought to aim at some good" is the opening sentence of Aristotle's Ethics.4 What is it for every action to aim at some good? We have the assumption here that every action is purposive and rational; purposive in that there is some goal which the agent sees as a good and rational in that performing the action is believed by the agent to be an appropriate means or way of bringing about the desired goal. This article presents the understanding of Eurasianism as an expression of Russia’s essence in the works of N.S. Trubetskoi, P.P. Suvchinskii, P.N. Savitskii, and L.P. Karsavin. We use the cognitive category “historical collective individuality” for a more complete and deeper understanding of Eurasianism as a set of views and approaches, as well as a certain specialized social community of its representatives. The use of this category allows us to reveal Eurasianism as an area of ideas expressing the essence of Russia. (...) N.S. Trubetskoi considers Russian-Turanian unity to be just such an expression of that essence; for P.N. Savitskii, it is the circle of “European and Asiatic/Asian cultures”; for P.P. Suvchinskii, Russia’s great testament is the unification of the people and the intelligentsia under the “all-resolving dome of the Orthodox Church”; and for L.P. Karsavin, Russia’s essence consists in its development as a Eurasian subject or a collective person. In addition, the category of historical collective individuality allows us to reveal Eurasianism as a “gold reserve of life” for Eurasians in exile. Thus, the unity of metaphysics and life sought in Russian philosophy is achieved. We draw conclusions about the historicity of the Eurasianist collective individuality. (shrink)

This article discusses development of the content of the concept “Philosophy Steamer,” which refers to the 1922 expulsion from Russia of a group of intelligentsia who sharply criticized the authorities. The author shows that the group of exiled philosophers was united both by their previous philosophical and social activity and by their joint activity as émigrés. She analyzes the concepts of “historical collective individuality,” “collective person,” and “communal person” introduced by Lev P. Karsavin in order to determine the holistic nature (...) of this exiled social group and its characteristic self-consciousness, behavior, aspirations, and feelings (moods). Examining the event known as the “Philosophy Steamer” through the concepts listed above allows the author to cognize the historical, social, and cultural reality more broadly and productively. The article demonstrates that the concept of a collective person can also be applied to other social groups, thereby extending the methodology of human-studies cognition beyond the study of facts and statistical data. It stands to reason that ekphrasis cannot be limited to the detailed but plain description of the artifact, for it often concerns what is not there in the painting as well. Authors of the ekphrases sometimes “animate” or “revive” a work of art, which manifests itself in narrativization of painted scenes. The process is also frequently supplemented by its continuation or backstory and fictitious utterances of figures depicted in the painting. The paper discusses a specific literary form of that kind (...) of “animation”, that is texts in which characters that can be seen in a painting receive the voice through the prosopopoeia and expose their selfawareness of “artificiality”. The precise subject of this study is P. Assouline’s novel Le Portrait which is narrated by baroness Betty de Rothschild, the figure from the J.A.D. Ingres’ painting. In this article, I am focusing on the mode and meaning of prosopopoeia in Assouline’s novel. I am also trying to examine whether every text that is based on the concept of a “speaking” work of art can be described as ekphrasis. (shrink)

While philosophy and psychoanalysis privilege language and conceptual distinctions and mistrust the image, the philosopher and psychoanalyst Julia Kristeva recognizes the power of art and the imagination to unblock important sources of meaning. She also appreciates the process through which creative acts counteract and transform feelings of violence and depression. Reviewing Kristeva's corpus, Elaine P. Miller considers the intellectual's "aesthetic idea" and "thought specular" in their capacity to reshape depressive thought on both the individual and cultural level. She revisits Kristeva's (...) reading of Walter Benjamin with reference to melancholic art and the imagination's allegorical structure; her analysis of Byzantine iconoclasm in relation to Freud's psychoanalytic theory of negation and Hegel's dialectical negativity; her understanding of Proust as an exemplary practitioner of sublimation; her rereading of Kant and Arendt in terms of art as an intentional lingering with foreignness; and her argument that forgiveness is both a philosophical and psychoanalytic method of transcending a "stuck" existence. Focusing on specific artworks that illustrate Kristeva's ideas, from ancient Greek tragedy to early photography, contemporary installation art, and film, Miller positions creative acts as a form of "spiritual inoculation" against the violence of our society and its discouragement of thought and reflection. The term "politics" often brings to mind "Big P" political matters, including citizenship, governance of the state, the election of officials, and the formation of public policy. Another way of thinking about politics is to associate it with power relations in social interactions of any kind. If one (...) thinks about it in the second way, then wherever there are two or more people, there are politics. Questions such as "Should we participate in politics?" and "How engaged should we be?" become meaningless. Michel Foucault's discussions of the pastoral forms of governance employed by the modern state, specifically the uses of technologies of the self, challenged long-held views about relationships between the individual and the state. In particular, his claim that major dominations are the effects, not the causes, of myriad local and particular mobilizations of power provides a rationale for examining these local mobilizations.1 If major dominations and "Big P" politics are the effects of local mobilizations, then everyone participating in social interactions participates in "Big P" politics, as well.This more global conception of political engagement stands in sharp contrast to a narrower one, which can be described using a "swimmer-on the-shore" metaphor. The swimmer stands on a beach looking out at the sea and decides whether this is a day for a toe dip, wading to the knees, total immersion, or staying out of the water altogether. The swimmer can choose to stay dry (unless inundated by the tide or a tidal wave). This conception is evident in a comment reportedly made by a music education professor in response to two back-to-back annual meetings of the music education researchers at Committee on Institutional Cooperation (CIC) institutions. Both meetings had been devoted to critique of MENC's policies and practices, and the disgruntled professor reportedly remarked, "That's enough about politics; let's get back to talking about music." In his view, which is shaped by the "swimmer" conception, some discussions are political, others are not; while some curricular areas, such as civics and sex education, are political, music is not, nor should it be. If, however, we assume that there is power in all social relations, then the "swimmer" conception has no salience because everyone is immersed; music, because it is a social activity, is political. Its engagement in politics is neither good nor bad, in and of itself; rather, it is inevitable. [End Page 168]Some may concede that although it is not possible to be disengaged, it is necessary and good for scholars and teachers to be politically "neutral." Conceptions of neutrality rely on a one-dimensional linear integers model of political engagement. On the line representing the integers, there is a plus side, a minus side, and zero, the zero presumably being the point of neutrality. People and views associated with the far ends of the line are extreme, while those closer to the zero point are moderate. In addition, positions on the line are assigned a moral value; depending upon the issue and the perspective from which the line is being viewed, plus can be good but minus is bad, plus can be bad but minus is good, neutral can be good, or neutral can be bad. If every issue and every social relation is political, however, and if we assume that people and relationships change, then the linear integers model is inadequate because it fails to embody the dynamic, multi-dimensional character of the complex social relations in which all of us engage. Another problem with the linear integers model is that it fails to interrogate the power vested in whomever or whatever decides what will constitute extreme, moderate, or neutral—what will be deemed reasonable and what outrageous. Sometimes this responsibility has been assigned to a fictitious "reasonable" person without much thought having been given to the political dimensions of the construction of reasoning. Objectives To determine the frequencies and types of limitation of medical treatment performed by physician members of the German Society for Palliative Medicine and to analyse the findings with respect to clinical and ethical aspects of end-of-life practices. Design Cross-sectional postal survey. Setting Data collection via the secretary of the German Society for Palliative Medicine using the German language version of the EURELD survey instrument. Subjects All 1645 physician members of the German Society for Palliative Medicine. Main outcome measures Types (...) and frequencies of limitation of treatment and possible determinants. Results 901 physicians participated in the study (response rate 55.8%). Participants reported limitation of treatment in 69.1% of cases. These decisions most often affected artificial nutrition (19%), chemotherapy (14%), antibiotics (11%) and medication other than antibiotics (11%). In the majority of eligible cases, physicians estimated the life-shortening effect of limitation of treatment to be <7 days. However, estimations differ depending on the medical measures in question. Bivariate statistical analysis indicated that withholding of treatment was performed significantly more frequently for patients aged ≥65 years (p=0.019). In addition, there were significant associations between the incidence of limitation of treatment and the different diseases reported by respondents as the underlying cause of death. Conclusion The findings of this study provide information on the current state of an ethically and clinically challenging aspect of clinical practice and can serve as a starting point for further interdisciplinary research on normative and empirical aspects of treatment decision-making at the end of life.
