A main objective of this study is to assess the opinions of 50 behavioral health patients on selective control over their behavioral and physical health information. We explored patients' preferences regarding current consent models, what health information should be shared for care and research and whether these preferences vary based on the sensitivity of health information and/or the type of provider involved. The other objective of this study was to solicit opinions of 8 behavioral health providers on patient-driven granular control (...) of health information and potential impact on care.Electronic surveys were implemented at an outpatient Behavioral Health facility that provides care for behavioral health patients with non-serious mental illnesses. The Patient Survey included questions regarding patients' demographics and about their concerns and preferences for data sharing for care and research. The Provider Survey included questions about their view on the current consent process and perceptions on barriers and facilitators to implement patient-controlled granular consent models.This novel study provides valuable preliminary data that can help guide future studies to better understand privacy choices of this underrepresented patient group. In lieu of an abstract, here is a brief excerpt of the content:Clinical CommentaryTor Phern Chern, Associate ConsultantThe case scenario describes a difficult clinical and ethical challenge of a psychiatric patient refusing treatment in the context of a treatment-resistant condition that may be affecting her capacity to refuse treatment.The patient described in the case scenario displayed partial treatment refusal as evidenced by her refusal of ECT and adherence with medication and her voluntary hospital admission. Partial treatment refusal is generally (...) more common than absolute treatment refusal1 and can be sporadic, time-limited, periodic or continuous. It is important for the medical team to clarify which category of treatment refusal this patient’s refusal falls under. These broad categories include the doctor-patient relationship, religion, secondary gain, treatment- or illness-based refusals.2 The proper identification of category of treatment refusal will guide subsequent management.Refusal of treatments may be in response to reality-based transference issues like fear, distrust or pressure from the patient’s family. In this particular case, there is little data about her relationship with her treating psychiatrist, but the lack of a trusting doctor-patient relationship could be a factor for treatment refusal. The treating psychiatrist at the inpatient setting could be different from the patient’s routine outpatient psychiatrist and this could result in the lack of a trusting doctor-patient relationship. The remedy for this would be to develop a trusting relationship between her treating psychiatrist and the patient via understanding the patient’s needs and concerns. A useful adjunct would be to identify someone with whom the patient has a trusting relationship and recruit that person to help develop trust in the doctor-patient relationship. On this point, it is interesting to note that the patient’s family was noted to be highly supportive and influential in previous treatment decisions, agreed [End Page 235] with the treating psychiatrist’s treatment recommendation and yet both the treating psychiatrist and the patient’s family were unable to convince her to accept ECT. One possibility is that the patient may see her family’s support of the treating psychiatrist’s recommendation as one reason to refuse treatment. This possibility warrants deeper explorations of her premorbid underlying cultural and religious beliefs and attitudes towards her family and her perceived role in her community.Religion-based refusals of treatment (e.g. blood transfusions for Jehovah’s Witnesses) are less prominent in psychiatry than in general medicine but may manifest in treatment refusal due to advice from religious leaders on the need for spiritual solutions to the patient’s challenges. Religion-based refusals need to be distinguished from an illness-based refusal but if truly present will require the support of appropriate religious leaders to allow the patient more management options other than spiritual support for her psychiatric disorder. There is no specific information about the patient’s religion and religion-based refusals of ECT are not particularly common so this is unlikely to be a major reason for treatment refusal in this case.Treatment-based refusals of care are related to the specific treatment modality being offered and are often motivated by concerns about the side effects of treatment or past traumas that the treatment elicits. In this particular case, the patient has demonstrated some degree of treatment resistance as evidenced by the relapse of her bipolar disorder despite adherence to medication, and a change in management is certainly warranted. However, a change in medication management may be possible before proceeding to ECT in view of the patient’s acceptance of medication and no clear evidence of violent or disturbed behaviour in the ward. The occasional refusal of food and water is worrying, but if the patient can be coaxed to continue oral nutrition, there is no absolute indication to have ECT for rapid treatment response. Other options that may be acceptable to the patient are to optimise the dosage of current medications, switch medications or add augmenting pharmacological agents before proceeding to ECT. A change in pharmacotherapy must take into account her past treatment response and side effects to individual medications. Thetranslation Chern-Simons type three-formcoframe∧torsion on a Riemann-Cartan spacetime is related (by differentiation) to the Nieh-Yan fourform. Following Chandia and Zanelli, two spaces with nontrivial translational Chern-Simons forms are discussed. We then demonstrate, first within the classical Einstein-Cartan-Dirac theory and second in the quantum heat kernel approach to the Dirac operator, how the Nieh-Yan form surfaces in both contexts, in contrast to what has been assumed previously.

Philosophers of physics and physicists have long been intrigued by the analogies and disanalogies between gravitational theories and gauge theories. Indeed, repeated attempts to collapse these disanalogies have made us acutely aware that there are fairly general obstacles to doing so. Nonetheless, there is a special case space-time dimensions) in which gravity is often claimed to be identical to a gauge theory. I subject this claim to philosophical scrutiny in this article. Despite its long history and stunning experimental successes, the mathematical foundation of perturbative quantum field theory is still a subject of ongoing research. This book aims at presenting some of the most recent advances in the field, and at reflecting the diversity of approaches and tools invented and currently employed. Both leading experts and comparative newcomers to the field present their latest findings, helping readers to gain a better understanding of not only quantum but also classical field theories. Weight systems on chord diagrams play a central role in knot theory and Chern-Simons theory; and more recently in stringy quantum gravity. We highlight that the noncommutative algebra of horizontal chord diagrams is canonically a star-algebra, and ask which weight systems are positive with respect to this structure; hence we ask: Which weight systems are quantum states, if horizontal chord diagrams are quantum observables? We observe that the fundamental gl(n)-weight systems on horizontal chord diagrams with N strands may be (...) identified with the Cayley distance kernel at inverse temperature beta=ln(n) on the symmetric group on N elements. In contrast to related kernels like the Mallows kernel, the positivity of the Cayley distance kernel had remained open. We characterize its phases of indefinite, semi-definite and definite positivity, in dependence of the inverse temperature beta; and we prove that the Cayley distance kernel is positive (semi-)definite at beta=ln(n) for all n=1,2,3,... In particular, this proves that all fundamental gl(n)-weight systems are quantum states, and hence so are all their convex combinations. We close with briefly recalling how, under our "Hypothesis H", this result impacts on the identification of bound states of multiple M5-branes. (shrink)

In a class of quantum gravity approaches it is indicated that our observable world emerges out of a fundamental structure that appears highly resistant to any clear spatial or temporal interpretation. In this work we are examining an analogue quantum system that appears to simulate such an unintuitive structure: the emergence of the so called topological phase of matter depicted by the Chern–Simons gauge theory. By investigating the proposed analogy from the lens of category theory, we offer a clear (...) interpretation of the way in which space and time act at the fundamental level. (shrink)

Unter den griechischen exegetischen Werken zu den Schriften des Aristoteles kommt dem von um 200 n. Chr. wirkenden Alexander von Aphrodisias (Bücher 1-5) und einem byzantinischen Gelehrten des 13. Jahrhunderts, bekannt als Pseudo-Alexander, (Bücher 6--?14) stammenden Kommentar zur Metaphysik des Aristoteles eine überragende Bedeutung zu. Der Wert des durch fundierte Kenntnisse der Philosophie des Aristoteles ausgezeichneten Kommentars ist sowohl für die denkerische Durchdringung der aristotelischen Metaphysik als auch für die Geschichte der griechischen Philosophie nicht genug hochzuschätzen. Den Denkern des Mittelalters (...) lag der Text aber nicht vor. Erst im Jahre 1527 machte der berühmte spanische Humanist Juan Ginés de Sepúlveda (1490?-1573) der breiten Öffentlichkeit den Kommentar zur Metaphysik erst zugänglich. Seine immer noch geschätzte, in gewissem Sinne auch unentbehrliche Übersetzung des Kommentars zu den ersten zwölf Büchern erstellte er aufgrund der Kollationierung der ihm zu Gebote stehenden Handschriften mit philologischer Gewissenhaftigkeit und ermöglichte auf diese Weise die immer noch nicht erforschte Rezeption des Textes in den folgenden drei Jahrhunderten bis zur ersten Edition des vollständigen griechischen Originals durch Hermann Bonitz 1847. Ein Nachdruck der 'editio princeps' dieser lateinischen Übersetzung, die 1561 zum fünften und letzten Mal erschienen ist, ist nicht nur ein Akt der antiquarischen Pietät, sondern kommt gerade im digitalen Zeitalter einer Forderung der Forschung nach. We present a reformulation of loop quantum gravity with a cosmological constant and no matter as a Fermi-liquid theory. When the topological sector is deformed and large gauge symmetry is broken, we show that the Chern–Simons state reduces to Jacobson’s degenerate sector describing 1+1 dimensional propagating fermions with nonlocal interactions. The Hamiltonian admits a dual description which we realize in the simple BCS model of superconductivity. On one hand, Cooper pairs are interpreted as wormhole correlations at the de Sitter (...) horizon; their number yields the de Sitter entropy. On the other hand, BCS is mapped into a deformed conformal field theory reproducing the structure of quantum spin networks. When area measurements are performed, Cooper-pair insertions are activated on those edges of the spin network intersecting the given area, thus providing a description of quantum measurements in terms of excitations of a Fermi sea to superconducting levels. The cosmological constant problem is naturally addressed as a nonperturbative mass-gap effect of the true Fermi-liquid vacuum. (shrink)
