Background The often poor prognosis associated with cancer necessitates empowering patients to express their care preferences. Yet, the prevalence of Advance Directives (AD) among oncology patients remains low. This study investigated oncologists' perspectives on the interests and challenges associated with implementing AD. Methods A French national online survey targeting hospital-based oncologists explored five areas: AD information, writing support, AD usage, personal perceptions of AD's importance, and respondent's profile. The primary outcome was to assess how frequently oncologists provide patients with information (...) about AD in daily clinical practice. Additionally, we examined factors related to delivering information on AD. Results Of the 410 oncologists (50%) who responded to the survey, 75% (n = 308) deemed AD relevant. While 36% (n = 149) regularly inform patients about AD, 25% (n = 102) remain skeptical about AD. Among the respondents who do not consistently discuss AD, the most common reason given is the belief that AD may induce anxiety (n = 211/353; 60%). Of all respondents, 90% (n = 367) believe patients require specific information to draft relevant AD. Physicians with experience in palliative care were more likely to discuss AD (43% vs 32.3%, p = 0.027). Previous experience in critical care was associated with higher levels of distrust towards AD (31.5% vs 18.8%, p = 0.003), and 68.5% (n = 281) of the respondents expressed that designating a “person of trust” would be more appropriate than utilizing AD. Conclusion Despite the perceived relevance of AD, only a third of oncologists regularly apprise their patients about them. Significant uncertainty persists about the safety and relevance of AD. Affinity maturation occurs by the introduction of multiple point mutations in the variable region gene that encodes the antigen binding site. This somatic hypermutation is restricted to immunoglobulin genes and occurs at very high rates. The precise molecular basis of this process remains obscure. The Abbé de Saint-Pierre, best known for his 'Project for Perpetual Peace', in fact left a much larger and more coherent body of political and moral writing, but it has been only partially studied. This book, the first systematic exploration of his entire corpus, offers a complete re-evaluation of this important author's contributions to the Enlightenment. From the first decades of the eighteenth century, Saint-Pierre set forth a pioneering vision of politics as the harmonisation of interests, anticipating Bentham (...) as a utilitarian. He imagines replacing the system of inherited power and clientele networks, which structured Old Regime society and determined the exercise of power under absolutism, with a rationalised, meritocratic and dynamic organisation. He argued for the political values of social utility and public good to take the place of the Christian ideals of perfection and the aristocratic ideals of personal charisma. As a deist seeking to reconcile morality and religion, Saint-Pierre argued that the search for salvation through active piety must also promote social justice and beneficence--and that only the indivisible power of a rationalised monarch, informed by competent elites, could carry out the reforms necessary to yield a government which would produce the greatest happiness for the greatest number. Saint-Pierre, thus, provided among the first arguments for an imposed welfare state, well before the sources more frequently associated with that idea--political economists, cameralists and the physiocrats."--Pages v-vi. This book offers an interdisciplinary analysis of the social practice of taste in the wake of Pierre Bourdieu¿s sociology of taste. For the first time, this book unites sociologists and other social scientists with artists and curators, art theorists and art educators, and art, design and cultural historians who engage with the practice of taste as it relates to encounters with art, cultural institutions and the practices of everyday life, in national and transnational contexts. Alors que la notion de form constants venait à s’appliquer virtuellement à toutes les figures entoptiques, comme l’avait prévu son auteur, dont les photismes synesthésiques, Cytowic a décrit et Carol Steen représenté ce que voient réellement des synesthètes visuels. L’une des caractéristiques d’œuvres de peintres synesthètes serait justement la présence de ces formes classées par Klüver. L’anthropologie avec la thèse de l’externalisation a (...) montré que certaines cultures ont repris les form constants perçues dans le premier stade de l’intoxication pour en faire les patterns de leur art. Les modèles physiologico-mathématiques permettent de localiser en V1 l’origine de production des photismes et d’expliquer leur morphogénèse. Mais une explication des circuits neuraux empruntés pour passer des différents types de déclencheur sensoriel à l’apparition de photismes synesthésiques reste aujourd’hui au stade de l’hypothèse. La synesthésie est l’objet de recherches alliant les sciences les plus contemporaines dont la génomique. Il est possible que les études sur la synesthésie transforment son statut de phénomène marginal en un élément essentiel dans la compréhension du système physiologique sous-jacent à l’esprit humain. In 1928, Klüver’s theory of form constants placed visual hallucinations into four broad categories. Whereas, as predicted by its author, the notion of form constants was virtually applicable to all entoptic figures, including synesthetic photisms, the very notion of entoptics was definitively extended. Cytowic has described and Carol Steen has represented what visual synesthetes actually see. The presence of these forms, as they have been classified by Klüver, is precisely one of the characteristics of the works of synesthetic painters. With the thesis of externalization, the field of Anthropology has shown that some cultures have adopted the constant forms perceived in the first stage of intoxication as patterns for their art. Physiological-mathematical models allow us to locate the origin of photism production in V1, and to explain these photisms’ morphogenesis. But, even today, an explanation of the neural pathways used to pass from different types of sensory triggers to the appearance of synesthetic photisms remains at the hypothesis stage. Synesthesia is a research topic that combines some of the most contemporary sciences, including genomics. Studies on synesthesia may just transform its status as that of a marginal phenomenon into that of an essential element in understanding the physiological system underlying the human mind. (shrink)

Skirting the Ethical offers highly original readings of six works, each noted for its politico-ethical stance. The first four (Sophocles' Antigone , Plato's Symposium and Republic and Hamann's "Aesthetica in nuce") have a recognized and honored place in the canon. The last two, Sebald's The Emigrants and Jane Campion's film The Piano , are exemplary for our contemporary scene. Nevertheless, the straightforward assumptions about justice, divine and state power, the good, and identity politics that every reader or viewer inevitably comes (...) upon are disrupted when one takes into account the role of language: both the way in which language is talked about and the way in which it performs. What emerges is a non-prescriptive ethics of another order that offers a resistance to power and simplistic conceptualizations of truth, an emancipation from the "must-be" that implies an ever-to-be-renewed renegotiation—a responsability that has much to do with the act of critique or interpretation. La modernité, dès ses débuts, attribue un rôle-clef aux passions : qu’elles soient hostiles à la Raison ou au contraire ses alliées, dangereuses ou fascinantes, elles marquent le rôle du corps, du désir, du langage et de l’imagination dans la nature de l’homme. La même époque voit se développer différentes variantes du matérialisme. Presque toutes réévaluent ce que la raison classique avait tendance à réprimer ou à considérer comme révélateur de la faiblesse humaine : le corps et tout ce qui, (...) dans l’âme ou dans la société, porte les traces de l’activité et de la positivité du corps. On peut donc s’attendre à ce que les matérialistes fassent un sort particulier aux passions, à ce qu’ils y reconnaissent des lois et non pas seulement des manques ou des vices, à ce qu’ils essaient d’en repérer l’efficace dans l’ensemble des activités humaines. Encore faut-il se demander comment chaque matérialisme procède, par quelle configuration propre il rend compte de ces phénomènes ou comment il dévie les discours de la théorie classique pour les forcer à passer par les objets qui sont les siens. Plutôt que de supposer l’existence d’une théorie matérialiste unique, une enquête s’impose donc qui prenne en vue la diversité de ces auteurs, les situe dans leur contexte et repère les points d’inflexion que rencontre chez chacun d’entre eux cette problématique générale des passions qui semble avoir gouverné plusieurs siècles. Pateman challenges the way contemporary society functions by questioning the standard interpretation of an idea that is deeply embedded in American and British political thought: that our rights and freedoms derive from the social contract explicated by Locke, Hobbes, and Rousseau and interpreted in the United States by the Founding Fathers. The author shows how we are told only half the story of the original contract that establishes modern patriarchy. The sexual contract is ignored and thus men's patriarchal right over (...) women is also glossed over. No attention is paid to the problems that arise when women are excluded from the original contract but incorporated into the new contractual order. One of the main targets of the book is those who try to turn contractarian theory to progressive use, and a major thesis of the book is that this is not possible. Thus those feminists who have looked to a more "proper" contract- one between genuinely equal partners, or one entered into without any coercion- are misleading themselves. In the author's words, "In contract theory universal freedom is always a hypothesis, a story, a political fiction. Contract always generates political right in the forms of domination and subordination." Thus the book is also aimed at mainstream political theorists, and socialist and other critics of contract theory. The author offers a sweeping challenge to conventional understandings- of both left and right- of actual contracts in everyday life: the marriage contract, the employment contract, the prostitution contract, and the new surrogate mother contract. By bringing a feminist perspective to bear on the contradictions and paradoxes surrounding women and contract, and the relation between the sexes, she is able to shed new light on fundamental political problems of freedom and subordination. En les lisant, on comprend ce qu'a pu représenter la découverte, pour un philosophe français, d'une pensée alors quasi inconnue, qui joua un rôle décisif dans son cheminement philosophique. On perçoit aussi une nouvelle dimension de la philosophie de Wittgenstein. Les deux premiers textes, consacrés au Tractatus logico-philosophicus, sont une (...) réflexion sur l'Indicible, à partir de la formule wittgensteinienne : " Ce qui s'exprime dans le langage, nous ne pouvons l'exprimer par le langage ". Les deux derniers, qui ont pour objet les Recherches Philosophiques, laissent entrevoir l'influence capitale qu'a eue sur Hadot la conception révolutionnaire du langage exprimée dans cet ouvrage ; les notions de jeu de langage et de forme de vie l'ont conduit à réfléchir sur la nature du discours philosophique : il n'y a pas "le" langage, ayant pour fonction de désigner des objets ou de traduire des pensées, mais des jeux de langage, destinés, entre autre, à produire un effet sur l'auditeur. Developing earlier studies of the system of numbers in Mundurucu, this paper argues that the Mundurucu numeral system is far more complex than usually assumed. The Mundurucu numeral system provides indirect but insightful arguments for a modular approach to numbers and numerals. It is argued that distinct components must be distinguished, such as a system of representation of numbers in the format of internal magnitudes, a system of representation for individuals and sets, and one-to-one correspondences between the numerosity expressed by (...) the number and its metrics. It is shown that while many-number systems involve a compositionality of units, sets and sets composed of units, few-number languages, such as Mundurucu, do not have access to sets composed of units in the usual way. The nonconfigurational character of the Mundurucu language, which is related to a property for which we coin the term 'low compositionality power', accounts for this and explains the curious fact that Mundurucus make use of marked one-to-one correspondence strategies in order to overcome the limitations of the core system at the perceptual/motor interface of the language faculty. We develop an analysis of a particular construction, parallel numbers, which has not been studied before, elucidating the whole system. This analysis, we argue, sheds new light on classical philosophical, psychological and linguistic debates about numbers and numerals and their relation to language, and more particularly, sheds light on few-number languages.
